at all possible, we recommend that you stop by Planned Parenthood
to pick up these materials in person or have them mailed
to you to avoid some of the problems with accessing them over
the public Internet. This State mandated law says the patient
has the option to review or not review these materials. However,
you must sign the informed consent form before your abortion
can be performed.
You
should know that the materials were created by the State of Michigan
through legislation drafted by Right
to Life of Michigan. While the State claims they are medically
accurate and unbiased, we do not necessarily agree. If you have

There
are about 25 pages on the website. You are not required to view
all of them. At a minimum, clicking on each section heading once
and viewing the information related to your procedure, the length
of your pregnancy, the prenatal, and parenting pages is required.
At the bottom of the "Parenting" page and click on continue.
Then click on "Informed Consent Internet Confirmation Form".
Print this and bring it with you to your appointment. We cannot
perform your abortion without this form.
This
form is good for only 2 weeks. You will be provided more
information at your consultation and any questions you have will
be addressed.
